What is the Alive Health Survey?
Alive Health is an Occupational Health & Safety survey, founded on global best practice models.

What is the theory foundation?
Alive Health is built on the theory of Salutogenesis. This is the study of the origins of health and focuses on factors that support human health and well-being, rather than on factors that cause disease (pathogenesis). In principle you can say that Alive Health focuses on doing more of what is good and causing a healthier future.

Is the survey only in English?
No, Alive Health is translated, and culturally adapted, to 45 languages.

Is it volunteering to take part in the survey?
Yes, it is up to you to take part in the survey.

Why should I do the survey?
The biggest benefits is that you will get a personal report about your own health & wellbeing status, as well as best practice tips for how to live better.

Does Alive Health provide me with a diagnosis?
No, Alive Health does not provide you with any form of diagnosis.
You will, however, receive scores - based on your answers – according to the respective models in the system. These scores are set by the creators / owners of the models, like the World Health Organization, or the Norwegian Technical University. If you are uncertain about your health, please contact your personal doctor.

Any tips in how to do the survey?
Make yourself a cup of tea or coffee and set aside enough time to do survey. In average, it takes about 20 minutes to complete. Be as honest as you can, and the rest is just fun.

What technical device is best?
Alive Health is available for all types of technical devices; PC, Mobile and Tablet.
The larger the device, the simpler it is to complete. That way, we recommend using a PC.

What is the Alive Health Platform?
The Alive Health Platform is nicked ‘The Hub!’.
This is a Triple A - Management System, related to health & safety.
The platform consists of: Assessments, Analytics, and a practical Academy.

What is the process for conducting the survey?
The process for conducting the survey is rather straight forward.
1) You will receive an invite from your employer (url or QR code)
2) You press the invite and register for the survey, as well as setting your security questions.
3) You then answer the survey as honest and truthfully as you can.
4) Finally, you will receive your personal report.

Who can see my report?
Nobody, but you, can see your report. The report is also protected by a personal password, that only you know. This password is generated from one of the security questions you selected.

How long will my private details be stored?
When you download your report, your private identifications are automatically deleted in the system. The same goes for the report.

Whet if I forget to download my report
If you forget to download your report when prompted, the system will store it up to a maximum of 48 hours. During this window, you will get three warnings. The first after 24 hours, then after 36 hours, and then finally 3 hours before deletion.

Why do I need to register for an anonymous survey?
Registering for the survey is necessary in order to securely facilitate the survey taking process. Granting you control over the survey taking process itself and severing any personal ties upon completion. It also allows you to access your report when done.

The registration generates a unique and personal survey token (url). To secure your personal token, you set security questions that must be answered before use. This way no one else is able to intercept, see or continue the survey on your behalf- even on the same computer or device.

If you register for the survey but do not complete it, you will receive follow up reminders until it is done. Your unique token allows you to continue the survey if left incomplete. This means that if the internet connection is severed, you will be able to pick up from where you left off.

 
If I log out, how do I log in again?
If you wish to continue the survey at a later time, or if the internet connection is cut, you simply click the personal survey url that was sent to your email. You will then be prompted to answer one of your security questions before you can continue.

The internet (wi-fi) connection was lost while I was in the middle of taking the survey, can I continue?
Yes, you simply click the personal survey url that was sent to your email. You will then be prompted to answer one of your security questions before you can continue.

Can any health / sensitive data be traced back to me?
No, you as a data subject (read: an individual) are non-identifiable, unreachable, and untraceable. Data points with fewer than 20 individuals will not be shown in the company report. The 20 pax limit has been set to uphold the HIPAA Compliance Criteria.

*The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that required the creation of national standards to protect sensitive patient health information from being disclosed without the patient’s consent or knowledge.

How are my responses stored?
Your responses (read: data points) are stored in a protected cloud and cannot be linked or traced back to you as a person. Whilst uploading your responses, your ID (name, email, IP address etc.) are one-way encrypted (non-reversable). As you exit the survey, any id-related information about you is, as informed about above, deleted.

 What are The Five Alive Health Tips?
The Five Alive Health Tips are best practice, and generic public health tips, linked to various health models. They are sound principles and have no side effects. As always, and whenever in doubt, consult your doctor for further assistance.

Will I receive unwanted emails after I’m done with the survey?
No, after the survey, any links to you, are deleted from the system. You will hence not receive any unwanted emails.
